Dynamic Mobile Dentistry – Digital Application Forms

Project Overview

Dynamic Mobile Dentistry required a secure digital solution for processing patient application forms — replacing traditional paper workflows with an entirely web-based system.
Patients could complete and digitally sign their forms directly on the site using either a mouse or touchscreen, and receive a signed PDF copy instantly by email.

The system had to reproduce existing official paper forms exactly, ensuring that the digital printouts matched every field, checkbox, and line position of the original government-style documentation.

Technical Implementation

Form Replication & Layout Precision

Three distinct paper templates were rebuilt in HTML and CSS from scratch.
Every label, text line, and input box was manually measured and coded to align perfectly with the original layout. Multiple iterations were performed until printed PDFs achieved 100% positional accuracy.

Signature Capture & Data Handling

• Implemented HTML5 Canvas signature fields compatible with both desktop and mobile browsers.
• Captured and serialized signature data alongside all input fields into the database.
• Triggered a post-submission event hook to inject the captured values into the correct HTML placeholders.

PDF Generation & Email Automation

• After form submission, the completed HTML template (with embedded signature) was converted into a PDF document.
• The system automatically emailed the applicant a digitally signed copy, while forwarding an identical copy to the admin team for record-keeping.
• The entire process executed seamlessly in under a few seconds.
